Respect: You have the right to be treated with dignity and respect. Services and Fees: You must be informed in writing about services and fees before you enter the nursing home. Money: You have the right to manage your own money or to choose someone else you trust to do this for you. Privacy: You have the right to privacy, and to keep and use your personal belongings and property as long as it doesn't interfere with the rights, health, or safety of others. Medical Care: You have the right to be informed about your medical condition, medications, and to see you own doctor. You also have the right to refuse medications and treatments.

It is a violation of state and federal law for any person, including facility staff, volunteers, visitors, family members or guardians, or another resident, to abuse or neglect a resident. If you suspect abuse or neglect, or if a resident tells you they are experiencing this problem, it is important to believe the resident and report the allegation immediately. This will help prevent further suffering by any resident. Many states have laws that require the reporting of abuse and neglect. Find out what your state requires. Put your report in writing, date it, and keep a copy. Convey as much information as you can about the situation. Provide as much background information as possible. A thorough report will help the investigator to address the situation quickly.

Definition:
A term used to refer to the mental and physical deterioration of old age.

Care/Case Management

Definition:
Care management is the process through which needs are identified and managed to allow the consumer to receive service in the most appropriate, least restrictive setting. This process focuses on consumer need and includes: development of a care plan addressing consumer need; explaining options for service delivery; ongoing monitoring of service delivery; periodically reassessing the consumer condition; making the necessary adjustments to the care plan; and notification to providers of changes in service provision.

The loss of voluntary control over bladder or bowel functions
